During the first few weeks of school, seniors will receive a folder containing the following forms. Seniors must turn in the Senior College Forms to the school counselor at least 2 weeks before the college application deadline. The sooner, the better. All forms are confidential and are only used for the purpose of helping the counselor write college recommendation letters. Remember, you will still be responsible for asking other teachers to write recommendation letters for college applications. Senior Autobiography: To be completed by the student
Parent Brag Sheet: To be completed by at least 1 parent/guardian
Teacher Recommendation Form: Seniors must give one teacher a recommendation form to fill out and return to the counselor. It is suggested that they choose high school teachers who knows them well. The teacher must be a core academic class (Math, English, History, and Science).
